Lapsed Donors & Annual Physicals

If it has been more than 4 months since your last donation, you must schedule an appointment for a physical to resume donating. Please use this registration form to schedule your donation physical appointment.

If you are a repeat donor and you are due for your annual physical, please use this page to schedule your annual physical appointment.

Please Read Before Scheduling

Donation appointments are only for new donors and donors that require a screening physical.  If you need to reschedule or cancel an appointment, please refer to the confirmation email that you received or contact us for assistance.

All repeat donors that do not need a physical should walk in to donate during regular donation hours. Repeat donors should not schedule an appointment unless they need a physical.

If you previously donated at ABO Plasma and it has been more than 4 months since your last donation, please contact us before scheduling.
